Current Patients/Family Login

If you are currently a patient, you can use this website for requesting a preventive exam, request a refill, communicate with your doctor, pay your bill, etc. at your convenience. If this is an emergency please call 911.  If you are wanting a Family Login please contact our office so appropriate authorizations can be obtained. 

New Patients

If you are visiting our office for the first time then you can complete your new patient paperwork in the convenience of your home.  Please contact our office at (541)779-5531 so you can receive a login ID and Password.
